St. Joseph's Health Centre is looking to recruit a Full-time Patient Navigator to join our wonderful team. We are CAR Accredited and an OBSP assessment site. In the last year we opened our brand new Breast Centre, The Geoffrey H. Wood Centre of Excellence in Breast Cancer Screening and Treatment.

This non-unionized position is one of a team of Technologists, Sonographers, Radiologists and Surgeons working within our beautifully renovated Breast Centre, delivering high quality services to meet the needs of our referring physicians and patients.

Duties and Responsibilities Include:

    • Perform all Mammography Technologist responsibilities to ensure a high quality diagnostic result

    • Consult with the Radiologist, and other interprofessional team members as required
    • Assist in inventory control, including ordering and restocking of supplies
    • Organization and prioritization of biopsy examinations and procedures in our RIS.

    • Assist Radiologist/Surgeons with biopsies and other procedures as required
    • Ensures the care provided is safe, appropriate, tailored, timely, and maximizes the potential of the available equipment and resources
    • Demonstrates commitment to patient and family-centered approach to care

    • Effective establishment of interpersonal relationships
    • Application of adult learning principles
    • Reviews referrals to assess and ensure completeness and coordinates obtaining any essential information for the consultation
    • Provides education and support regarding required diagnostic tests
    • Provides patient and family support and education during waiting times to surgical and plastic consults and alters appointments/consults in collaboration with the clerical team
    • Interpreting pathology reports to ensure appropriate follow-up is documented and performed
    • Communicate abnormal exam results to the patient and ordering physician for all patients
    • Responsible for monitoring Breast Imaging bi-monthly statistics for departmental examination wait-times

    • Respects patient rights and needs, and foster an environment that encourages the patients' involvement in their own care. This commitment is extended to patients from all cultures and backgrounds, ensuring all are treated with dignity, equity and respect
    • Participates in quality control, assurance and improvement programs

    • Monitors patient status and responds to any change in condition, to ensure patient safety and comfort

    • Must be able to work independently and without supervision
    • Prioritizing urgency of Emergency breast exams/procedures to ensure timely patient care
    • Monitoring own self-progress against established priorities and deliverables and taking corrective action, as required
    • This position requires an individual with strong analytical, interpersonal and problem solving abilities as well as the ability to prioritize and multi-task
    • This individual will work with a high degree of autonomy with respect to time management, work direction and task completion
    • Change and innovation planning
    • In collaboration with the front line team, Clinical Manager, Clinical Operations Leader, Physicians and others, the Navigator works to ensure safe, current and where possible best practices are being used in patient care
    • Group facilitation, strategies for consensus building, conflict resolution, coaching/mentoring
    • Other duties as assigned and required for operational purposes
    • Shifts are mostly days but could include evenings and weekends.

    Qualifications and Skills:

    • Registered with CMRITO required.

    · Navigator must have diploma or undergraduate degree in a recognized Medical Radiation Sciences Program.

    · Minimum 3 years recent experience as a Mammography Technologist required.

    · Patient Navigator Certificate from a recognized Program completed or in-process of completion upon application.

    · Current CPR certification required.

    · CAMRT registration an asset.
