Job Description

Reports to: Manager
Hours: 37.5 hours per week
Salary: $44.51 - $55.64 per hour
Status: Temporary Full-Time
Closing Date: January 23, 2025

As a member of Peter Munk Cardiac Centre team, the Cardiac Triage Nurse Coordinator facilitates referrals for patients requiring interventions in the Cardiac Catheterization Labs. The Triage Coordinator reports to the CVIU and Cardiac Triage Nurse Manager and works alongside fellow coordinators, appointment schedulers and administrative assistants while liaising with referring physicians and surgeons and cath lab staff. The successful applicant will use critical thinking, knowledge of procedure protocols to assign and schedule patients/procedures to ensure efficient use of available cath lab time.

Duties and responsibilities include:

  • Reviews referral packages for completeness as to procedures to be performed; completion of lab work and pre-admission tests, special equipment or interdisciplinary team requirements and EPIC documentation as per established department standards of practice, policies and procedures.
  • Communicates changes in patient's condition and other urgent information to the interventionalists in a timely manner
  • Facilitates referrals for patients requiring percutaneous interventions and diagnostic caths
  • Liaise with a wide array of administrative and clinical team members throughout the organization and referring physicians/organizations
  • Reviews cases with appropriate specialty physicians
  • Prioritizes and triages patients for procedures according to urgency
  • Ensures that patients receive timely access to cardiac procedures as per CorHealth and Ontario Health benchmarks
  • Ensures all necessary tests are scheduled and completed so patients can have procedures in timely manner
  • Accurate and complete documentation of all communications
  • Provides information to facilitate physician to physician communication
  • Provides patients and families with education and support while waiting for procedure
  • Applies best practice guidelines to ensure that patients receive care according to provincial standard
  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality and safety standards
  • Promote and maintain a positive and calming working relationship with team members and a healthy work environment
  • Participates in continuous quality improvement activities within University Health Network to enrich and advance processes through the effective use of resources and systems to deliver optimal patient care.
  • Performs cross-functional responsibilities and other duties consistent with the job classification, as assigned

Qualifications

  • Current registration with the College of Nurses of Ontario as a Registered Nurse
  • Bachelor of Science in Nursing; Masters preferred
  • Current BCLS Certification
  • Minimum Three (3) to Five (5) years Cath Lab, Cardiology/C.V. Surgery Nursing and/or CICU/CVICU
  • Strong organizational and time management skills
  • Excellent interpersonal and customer service skills
  • Excellent computer skills including Microsoft Office and EPIC applications
  • Excellent verbal and written communication skills
  • Demonstrated ability to cultivate and build relationships
  • Strong track record of high-quality work and engagement is essential
  • Able to adapt to a constantly changing environment that requires ongoing revaluation of priorities
  • Demonstrated ability to use good judgment in assessing difficult situations
  • Able to work collaboratively within a group setting
  • Proven track record of taking initiative
