St. Joseph's Health Centre site is looking for a highly motivated and self-directed Nurse Counsellor Diabetes Education to join its dynamic interprofessional team as part of the Acute Care Team. St. Joseph's Health Centre serves a complex population of patients living with diabetes. The Nurse Counsellor works within the acute care setting providing diabetes education to patients and staff. The Nurse Counsellor Diabetes Education works within a changing environment and in inter-professional teams to advocate for and promote wellness while ensuring the delivery of safe, effective and ethical nursing care. The successful candidate will be responsible to provide excellent care to a diverse acute medical patient population in a fast-paced setting.
The RN shares their knowledge and expertise with others and contributes to the advancement of nursing practice excellence. The RN continually enhances their own practice and competencies through ongoing learning, education, experience and reflective practice.
Primary Responsibilities
· Acts as an inpatient resource for patients that have a new diagnosis of diabetes or are experiencing challenges with diabetes management
· Conducts thorough nursing assessments of clients with diabetes, taking into consideration ethno-cultural backgrounds and the social determinants of health
· Uses a variety of teaching methods and principles of adult education to engage with clients to enhance diabetes self-management
· Advocates on behalf of clients with respect to systemic issues as they arise
· Communicates effectively with patients, families and all members of the healthcare team.
· Practices collaboratively with all members of the healthcare team
· Acts as a diabetes resource to peers as needed
· Ensure continuity of care by maintaining up to-date medical records, communicating with the client's primary care provider and other service providers, and making referrals where appropriate
· Assists in the development and implementation of the Plan of Care and evaluates its ongoing effectiveness.
Qualifications
· Registered and in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse
· Current BCLS certificate required
· BScN preferred
· Certified Diabetes Educator (CDE) required, or must complete within one year of hire
· Certificate in adult education an asset
· Minimum of 1-2 years recent clinical diabetes experience required
· Experience adjusting insulin as per a medical directive required
· Experience with insulin pump therapy and troubleshooting required or must learn within one year of hire.
· Certified Pump Trainer an asset
· Excellent physical assessment skills
· Demonstrates problem solving, decision making and critical thinking skills
· Demonstrates excellent organizational and time-management skills
· Demonstrates initiative and the ability to function both independently and as an interprofessional team member in a fast-paced demanding environment
· Demonstrates excellent interpersonal and communication skills
· Good working knowledge of Electronic Medical Record an asset
· Excellent attendance record required
