Nurse Counsellor Diabetes Education (Job ID: 8932)

St. Joseph's Health Centre site is looking for a highly motivated and self-directed Registered Nurse (RN), Certified Diabetes Educator (CDE) to join its dynamic inter-professional team of endocrinologists and diabetes educators working in the Diabetes Management Program. St. Joseph's Health Centre serves a complex population of patients living with Diabetes and chronic disease. The RN,CDE works within the outpatient setting providing education to patients with type 1 diabetes (on MDI or insulin pumps), type 2 diabetes with complex medical needs and individuals with diabetes in pregnancy.. The successful candidate will be responsible to provide excellent care to a diverse and complex patient population in a fast-paced setting.. As part of the diabetes management program the nurse counsellor will work independently and collaboratively in the diabetes clinic, 1:1 appointments and group education setting.

The RN shares their knowledge and expertise with others and contributes to the advancement of nursing practice excellence. The RN continually enhances their own practice and competencies through ongoing learning, education, experience and reflective practice.

Primary Responsibilities RN,CDE

Work in our outpatient clinics providing services in individual, group and virtual care settings.

Provide collaborative comprehensive assessment and education for patients and families

taking into considerationthe social determinants of health.

Uses a variety of teaching methods and principles of adult education to engage with clients to enhance client self-management including insulin titration.

Communicates effectively with patients, families and all members of the healthcare team

Management of patients on an insulin pump and continuous glucose monitor technology

Ensures continuity of care by maintaining up to-date medical records, communicating with the clients primary care provider and other service providers, and making referrals where appropriate

Advocates on behalf of clients with respect to systemic issues as they arise

Assists in the development and implementation of the Plan of Care and evaluates its ongoing effectiveness.

Qualifications

Registered Nurse in good standing with the College of Nurses of Ontario (CNO)

Certified Diabetes Educator (CDE) certification required

Current BCLS certificate required

Minimum of 3-5 years recent experience in a Diabetes Management/Education Program

Experience with Continuous Glucose Monitor (CGM) technology including sensor insertion/training and analysis/interpretation of reports

Experience with insulin pumps including working knowledge of current insulin pumps available in Canada and interpretation of reports

Demonstrates problem solving, decision making and critical thinking skills

Demonstrates excellent organizational and time-management skills

Demonstrates initiative and the ability to function both independently and as an interprofessional team member in a fast-paced demanding environment

Demonstrates excellent interpersonal and communication skills

Good working knowledge of Electronic Medical Record eg EPIC an asset

Excellent attendance record required
