Join our St. Michael's Hospital team as we work to put patients and families at the center of everything we do, in the role of Nurse Practitioner - Stroke (Medicine) Program.

The Stroke (Medicine) Program at St. Michael's Hospital brings together the best of clinical care and research in one setting. The team provides exceptional, patient-centered care in a collaborative and integrated manner. An inter-disciplinary team of neurologists, nurses, pharmacists, social workers, neuropsychologists, physiotherapists, occupational therapists, speech therapists, neuropsychiatrists, physiatrists, and administrative staff support our Centre. The NP will work collaboratively with the team, reporting directly to the Senior Clinical Program Director. As a part of an academic centre, our team participates in ongoing research, quality improvement and professional development initiatives. This role is full-time on-site at our clinic

The NP works within 4 domains of practice:

Clinical Domain:

Demonstrates the NP competencies in providing clinical care to patients and their families as the central part of the NP role. The NP works autonomously to diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures within the legislated scope of practice and a defined Unity Health Toronto Practice Agreement.

Leads the interprofessional team to provide clinical care for patients in the Stroke (Medicine) Program, including making independent decisions for patient treatment and discharge, including completion of discharge summaries and transfer of accountability to community care providers

Knowledge Translation:

Analyzes, appraises and synthesizes research and best practices to use as a foundation for enhancing the delivery of quality patient care.

Uses knowledge translation techniques in innovative ways to put research-based knowledge into daily practice and to support and educate other members of the health care team.

Contributes to the dissemination of nursing knowledge through presentations and publications.

Scholarship:

Contributes to the culture of discovery at St. Michael's/Unity Health Toronto by advancing and/or appraising nursing research.

Evaluates outcomes of practice, practice patterns and systems of care against local and national benchmarks to identify variances in practice outcomes and population trends.

Identifies issues related to clinical practice and patient care and develops innovations to improve care and care delivery for patients, families, and communities.

Designs, directs, participates in and evaluates quality improvement methodologies to promote safe, timely, effective, efficient, equitable, and patient-centered care.

Leadership:

The NP acts as a transformational leader by using ideals, inspiration and intellectual stimulation to influence the behaviours and attitudes of others towards the goal of excellent patient care

Acts as a consultant and resource to members of health care team and external groups in the process of decision-making strategic planning and problem solving by committee involvement, participation in program development and evaluation

Qualifications:

· Masters Degree in Nursing/Nurse Practitioner (MN-NP) or Masters of Science in Nursing with successful completion of a post-Masters Acute Care Nurse Practitioner Certificate Program from a recognized school of graduate studies required

· Current College of Nurses of Ontario registration in the Extended Class (EC) as an NP (adult) or NP (primary health care) required

· Current Advanced Cardiac Life Support (ACLS) and Basic Cardiac Life Support (BCLS) certification required

• 1-3 years of demonstrated experience as a Nurse Practitioner required

· 3-5 years of relevant Acute medicine or Primary Care experience preferred

· Completion of CNA Certification in Neuroscience is an asset

· Recent and proven experience in managing patients with neurological conditions is an asset

· Demonstrated strong communication and interpersonal skills, with the ability to work collaboratively with physicians, inter-professional team members, and other stakeholders

· Motivated self-starter with the ability to work independently and collaboratively as part of a team
