Position summary

The Nurse Practitioner (NP), having a specialty-based focus, functions in an expanded nursing role demonstrating a high level of autonomy and expert skill to formulate clinical decisions and appropriately manage acute/chronic illness and promote wellness.

As a member of the Red Blood Cell Program team, the NP will specialize in complex pain management for patients with sickle cell disease. This will occur in both the outpatient and inpatient settings. The goal of inpatient support will be as a clinical expert as well as provide support for the safe and seamless transition from inpatient to outpatient care. The NP will work closely and be supported by chronic pain teams including the clinical teams for chronic pain at TR and CAMH.

The NP will demonstrate excellent understanding of the inequities from an intersectional lens including but not limited to discrimination, racism, Anti-Black racism, Islamophobia and xenophobia encountered by populations experiencing inherited blood disorders - particularly Sickle Cell Disease - and will demonstrate experience working with these populations and respective communities to address these inequities and improve patient experiences and outcomes. Recognizing that patients who access this clinic are from diverse cultural and ethno-racial heritage and socioeconomic circumstances, outstanding communication skills and working experience with social justice and health equity frameworks will be important characteristics for the successful candidate.

Duties

  • Manage and support complex pain management needs of SCD patients in the outpatient setting
  • Act in consultant role when patients admitted to the inpatient units at UHN for a complicated vaso-occlusive crisis.
  • Provide continuity of care at the time of discharge from hospital, supporting the patient's recovery at home and bridging care to the outpatient team and community resources.
  • Collaborate with community based pain management programs to help patients with pain relief that extend beyond the scope of the RBCD outpatient clinic.
  • Lead scholarly work in quality improvement, research and interprofessional education.
  • Play a leadership role and perform quality improvement and administrative responsibilities;
  • Participate in policy and procedure development;
  • Perform research activities; deliver, coordinate and participate in education and professional development activities/programs;
  • Perform activities to further develop and strengthen the profession;
  • Act as a resource and serving as a consultant to individuals and groups within the professional community and other hospital/agencies;
  • Perform other duties consistent with the job classification, as required.

Qualifications

  • Completion of Masters Degree Program in Nursing
  • Current College of Nurses of Ontario Registration in the Extended Class (EC) as a NP (adult) or NP (Primary Health Care) required or pending
  • Three to five years nursing experience in an acute care setting preferred
  • Experience in acute and chronic pain management is an asset
  • Demonstrated expertise as a Nurse Practitioner preferred
  • Lived experience as a member of an equity seeking group is an asset
  • Evidence of professional malpractice insurance
  • Knowledge of data management programs an asset
  • Effective verbal and written communication skills

  • Current certification in Basic Cardiac Life Support required
