Position Summary

To advocate for these patients, UHN established the new Gattuso Center for Social Medicine to transform how we deliver care for marginalized populations.
UHN is seeking a Nurse Practitioner to join our Social Medicine Program. This role will focus on addressing the social determinants of health for patients with unmet complex medical and social needs. The Social Medicine Program serves high-utilizing patients within the UHN Emergency Department and other units. As a member of the team, the Nurse Practitioner will collaborate with the Social Medicine Community Health Worker/ Social Medicine Navigator, UHN care teams, and community partners to deliver care that meet patients where they are.
Duties
· Autonomous Patient Care & Most Responsible Provider (MRP): As the MRP, you will manage a caseload of patients with complex multidisciplinary needs. You will be diagnosing, ordering and interpreting diagnostic tests, developing treatment plans, reviewing, reconciling, and prescribing medications, ensuring continuity of care, and performing procedures within the legislative scope of practice and defined collaborative practice agreement.
· Advanced Health Assessment & Management: Provide comprehensive health assessment, diagnosis, therapeutic management, health promotion, and prevention of illness and injury to patients.
· Develop Care Plans: Use advanced clinical knowledge to analyze data and synthesize information using a variety of framework or theories to develop care plans.
· Collaboration & Communication: Work closely and coordinate with physicians, specialists, and members of an interdisciplinary team to provide patient-centred care, support escalation and continuity of care in both hospital and community settings. Facilitate therapeutic communication and compassion with individuals. Ensure timely documentation in the electronic medical record.
· Mentorship & Leadership: Serve as a mentor to members of Social Medicine care team, foster and environment that promotes safe and collaborative care. Lead by example in management of clinical care, act as a clinical subject matter expert, and role model for the team.
· Research & Program Development: Contribute to advancement of Social Medicine care model through participation in research, program evaluation, policy development, advisory boards, conferences, and publishing academic work; and lead QI projects as needed.
· Community Engagement & Advocacy: Works alongside community organizations, City and Province to advocate for marginalized populations and improve access to care for patients.
