Salary: $51.54 - $64.43 per hour (To commensurate with experience and consistent with UHN Compensation Policy)
Hours: 37.5 hours per week
Shifts: Days (and/ or other based on program needs)

Position Summary

University Health Network (UHN) is seeking an experienced Nurse Practitioner (NP) to support the newly established NPH Program in an ambulatory care setting. This role provides a unique opportunity to deliver comprehensive outpatient care for neurology patients, including clinical oversight, diagnostic assessments, and care navigation. The NP will collaborate closely with physicians and the interprofessional team to manage care from patient referral through to follow-up and surgery coordination, ensuring seamless service delivery. They will also perform direct clinical care such as lumbar punctures

Duties

  • Provides clinical oversight during patient clinic visits, ensuring high quality, patient-centered care.
  • Conducts thorough patient history intake and performs initial clinical assessments.
  • Manages the review and triage of patient referrals to ensure timely care delivery.
  • Performs lumbar punctures and manages care protocols around these procedures.
  • Coordinates follow-up imaging or diagnostic tests as required to guide treatment decisions.
  • Prescribes medications and provides comprehensive patient education on treatment plans, both in-person and over the phone.
  • Reviews and interprets diagnostic reports (e.g., imaging, gait analysis) to inform clinical recommendations.
  • Collaborates with interdisciplinary team members, (e.g. Kinesiologists, neurologists, inpatient NP) to support patient assessments and align with care plans.
  • Supports clinical coordination duties, such as scheduling imaging and coordinating surgeries as required.
  • Coordinates the referral process to ensure timely transitions between providers and services.
  • Acts as a liaison between patients and physicians, offering support as needed.
  • Conducts follow-up appointments to monitor patient progress and adjust care plans.
  • Supports other care areas as needed and required.
  • Guide patients in identifying and accessing relevant community resources to support their care and well-being.

Qualifications

Qualifications

  • Completion of a Master's Degree in Nursing.
  • Registered Nurse (EC) certification complete or pending.
  • Nurse Practitioner (NP) Adult preferred, with active or pending registration in good standing with the College of Nurses of Ontario.
  • Minimum of three to five years of nursing experience in outpatient care or a closely related specialty, with a strong preference for experience in neurology.
  • Strong clinical skills in assessment, referral management, and performing lumbar punctures.
  • Experience with coordinating patient care, including referrals, follow-ups, and surgery scheduling.
  • Proficient in interpreting neuro-imaging and diagnostic reports.
  • Excellent interpersonal, organizational, and problem-solving skills.
  • Strong verbal and written communication abilities, with a focus on patient education.
  • Ability to work both independently and within an interdisciplinary team.
  • Commitment to evidence-based practice and continuous professional growth.
  • Proficient in clinical documentation and the use of healthcare information systems. UHN uses Epic and a requirement of the role is proficiency with Epic once hired and on boarded.
