St. Michael's Geriatric Mental Health Outreach Program is committed to providing specialized psychogeriatric consultation and follow up services to residents living in long term care homes with severe mental illness and/or behavioral response issues.

DUTIES & RESPONSIBILITIES:

  • Provision of Occupational Therapy to promote, maintain or restore health and wellbeing through engagement in meaningful occupations in the areas of self-care, productivity and leisure. Occupational therapy services will include assessment, treatment, consultation, discharge planning and education to patients, family members/caregivers, external agencies and SMH staff.
  • Provision of Occupational Therapy to residents living in long term care homes and St. Michael's Inpatient Mental Health Unit.
  • Able to work effectively as a member of an interprofessional team, collaborating with other disciplines to provide comprehensive patient care.
  • Contributes to the academic mandate of the organization as an educator, resource or support for staff, students and members of the academic and professional community.
  • Identifies and addresses educational needs for patients and their families.
  • Demonstrates commitment to ongoing professional development in order to continue to provide optimal patient care.
  • Is committed to, and participates in, ongoing quality improvement in the organization.
  • Provides patient care in accordance with all professional and organizational standards.

QUALIFICATIONS:

  • Must have a Master's degree (or equivalent) in Occupational Therapy
  • Must be currently licensed by and in good standing with the College of Occupational Therapists of Ontario
  • Current Cardiopulmonary Resuscitation Certification
  • Minimum three years recent experience in a mental health setting
  • Excellent communication, critical thinking and interpersonal skills
  • Proven flexibility and adaptability in a fast-paced environment
  • Excellent time management, organizational and prioritization skills as relates to patient care
  • Demonstrated commitment to and involvement with own profession (e.g. membership with professional association, etc.)
  • Demonstrates high level energy and initiative and excellent problem-solving and decision making skills
  • Demonstrates excellent ability to work with older adults with severe mental illness and/or behavioral response issues
  • Evidence of excellent work and attendance record.

HOURS: Monday to Friday, 7.5 Hour Days

Must be willing to work evenings and/or weekends as directed by the Program.
