Hours: 37.5 hour per week
Salary: $ 42.21 - $ 52.76 per hour (To commensurate with experience and consistent with UHN compensation policy)
Status: Permanent Full Time

POSITION:
IMPACT is an Assertive Community Treatment (ACT) program that provides team treatment, psychosocial rehabilitation and support in the community to adults with severe and persistent mental illness and a primary diagnosis of a primary psychotic disorder. Its interdisciplinary team includes Psychiatrists, Psychiatric Nurses, Social Workers, Community Mental Health Worker, Peer Support Worker, Psychiatric Resident, as well as students from different disciplines. IMPACT strives to minimize the impact of mental illness on clients' lives, by facilitating the integration of clients into communities of their choice, and supporting them to improve the quality of their lives, in the areas of health, housing, finance, and vocation.

DUTIES:

  • Primary case coordinator for a caseload of 10-12 clients
  • Conduct individualized OT assessments - functional, vocational, leisure skills
  • Contribute to the development of interdisciplinary treatment plans
  • Facilitate clients' skill development of self-care, ADL, and leisure activities in order to support clients' capacity for independent living
  • Provide crisis intervention and advocacy, based on clients' needs
  • Establish & maintain therapeutic relationships with clients, their family members, and significant others
  • Participate in daily interdisciplinary clinical rounds

Qualifications

  • Completion of a Master's Degree in Occupational Therapy
  • Registration with the College of Occupational Therapists of Ontario is required
  • Membership in the Ontario Society of Occupational Therapists is preferred
  • Minimum of 3 years working in mental health community setting
  • Strong knowledge of the ACT model and psychosocial rehabilitation principals
  • Strong knowledge of concurrent disorders and harm reduction approach
  • Strong knowledge of trauma-informed care
  • Training in and experience delivering individual psychotherapeutic modalities (i.e., CBT, DBT, Behaviour Activation, coping strategies for stress), considered an asset
  • Expertise in crisis assessment and management
  • Experience and ability to effectively work as a member of an interdisciplinary team in a health care setting
  • Excellent interpersonal skills
  • Excellent verbal and written communication skills
  • Excellent organizational and time management skills
  • Excellent problem solving skills
  • Knowledge of policies, legislation, programs and issues related to mental health services, social services, community services, and placement services relevant to adults
  • Valid driver's license and access to a reliable vehicle is necessary
