The Toronto Grace Health Centre (TGHC) provides, specialized care to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Transitional care, and Palliative Care services. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 3 sites – the Main site (Church & Bloor St) and our Harbour Lights site and Specialized Care Centre (Victoria Park & McNicoll). Position Summary:

The Patient Care Manager (PCM) is an integral member of the inter-professional team and is responsible for the clinical operations of the Post Acute Care Rehabilitation unit and accountable for the 24 hour delivery of care consistent with the philosophy, goals, policies, and guidelines of the Toronto Grace Health Centre.

The PCM utilizes the principles of management and evidence based best practices and processes to achieve optimal care. The primary functions are to manage patient flow and the efficient utilization of resources (human resources, fiscal and materials) on the unit. The PCM is a leader, coach and mentor for nurses, allied health professionals and administrative staff on the unit.

Main Responsibilities

Demonstrate a clear understanding of the Mission, Vision and Philosophy.

Take initiative and responsibility for decisions as an individual and exhibit commitment to personal and organizational goals.

Participate in the ongoing development of competencies, abilities, technology and the resources to achieve high standards of efficiency and effectiveness.

Directly supervise employees, responsibilities include interviewing, hiring and training employees; planning, assigning and directing work; evaluating performance; counsel employees when necessary regarding performance concerns; as well as addressing complaints and resolving problems.

Promote patient focused care and ensures that care and service are developed with the goals of the patient/family in mind.

Integrate the work efforts and goals of all team members.

Adhere to the Code of Conduct. Treat employees, patients and families with respect.

Promote a safe and efficient working environment by adhering to organizational policies, guidelines, infection control and standard precautions.

Assign, supervise and guide staff in providing high quality care and services including accurate documentation and participation in patient case conference meetings based on clinical competence.

Maintain confidentiality of all organizational and patient related information. Protect patient rights.

Maintain proper attendance and punctuality to ensure that the unit is operated in an efficient and cost effective manner.

Perform on-going review of clinical records of new, active and discharged patients.

Qualifications and Education:

Master’s degree in one of the required Health Professions; in Nursing preferred.

Current Certificate of Competence issued by the College of Nurses’ of Ontario in good standing

Minimum of 3-5 years working in a hospital environment; minimum 2 years of experience leading a health care team

Experience in General Internal Medicine, post-acute care low tolerance rehabilitation an asset

Experience with community based programs and services preferred

Self-directed; ability to handle a wide variety of administrative tasks

Well-organized with an ability to multi-task, prioritize and meet deadlines

Superior interpersonal skills and high degree of emotional intelligence

Proven change management skills combined with the ability to work with others

Proven ability to promote an academic practice environment within the clinical setting

High degree of professionalism, integrity, maturity, discretion, initiative and judgment

Self-motivated, team player, and ability to thrive in a fast paced environment.

Excellent oral and written communication skills

Demonstrated proficiency in Microsoft Office, advanced knowledge of Word, PowerPoint, Outlook, Excel)

High level of integrity, leads by example and exemplifies our core Organizational Values

BCLS Certification

Excellent attendance record

Must have physical strength, flexibility and endurance required to perform the job, as determined by the physical needs assessment of the position conducted by Occupational Health Services

Demonstrate membership within a professional association and/or professional interest groups related to practice preferred.

Client service oriented, with the ability to effectively work within a framework that supports diversity, inclusiveness, and equity.

Demonstrate respect and support for diversity within and among our patients, families, and teams.
