The Toronto Grace Health Centre (TGHC) provides specialized care and services to those individuals who require Complex Continuing Care, Post Acute Care Rehabilitation, Palliative Care, and Transitional Care. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities.

We are looking for a self-motivated and highly engaged Pharmacy Lead to join our Inpatient Pharmacy team. This is a unique and exciting opportunity to work in close proximity with complex patients, caring health care professionals, and hospital staff as we provide services to our growing outpatient patient base and transitional residential beds in the community.

POSITION SUMMARY

The Pharmacy Department is responsible for procuring, proper storing and controlling the distribution of drug products and drug information throughout the hospital, promoting the safe and appropriate usage of medications, providing clinical monitoring of individual patient drug therapies as well as manufacturing preparations and solutions. The department is also responsible for keeping abreast of current information and current changes in official regulations through continuing education and through official publications. The Pharmacy Lead assumes professional responsibility for the Pharmacy and actively participates in the provisions of an essential service of the hospital under the guiding principle of optimizing patient care.

QUALIFICATIONS

Licensed to practice pharmacy in Ontario as Part A Registered Pharmacist (RPh).

Bachelor of Science in Pharmacy (BScPhm) or Doctor of Pharmacy (PharmD).

Knowledge of Provincial and Federal legislation governing all aspects of drug use.

Minimum two years' relevant experience in hospital pharmacy practice preferred.

Commitment to continuous improvement and learning.

Demonstrated resilience to adapt in an ever-changing environment.

Possess initiative in providing high quality, patient-centered, pharmaceutical care for a diverse and medically complex patient population.

Strong communication and interpersonal skills in order to effectively facilitate the delivery of pharmacy services in a wider institutional setting.

Able to work independently to support safe and efficient pharmaceutical care.

Highly motivated, compassionate individual able to work closely with patients and their family members, physicians, nurses, and other Allied Health professionals using an interprofessional approach to providing patient care.

Knowledge of drug distribution systems, including blister packaging, unit dose, smart medication dispensing devices, e-reminders, etc.

Excellent attendance record.

High degree of accuracy and attention to detail.

Ability to meet bone fide occupational requirements as outlined in the Job Demands Analysis.

MAIN RESPONSIBILTIES

Assisting the Manager of Pharmacy in the planning and directing the Pharmacy Services so that it is in accordance with the Standards of Practice for Hospital Pharmacies and the Standards of Practice for Pharmacists as well as relevant legislation.

Assisting in developing and maintaining departmental policies and procedures to ensure that the Standards of Practice for Hospital Pharmacies: Pharmacy Services; Facilities, Equipment, Supplies and Information; Drug Procurement and Inventory Management; Medication Distribution; Return of Medications; Repackaging of Medications; and Medication Preparation are met, as well as developing policies and procedures that are consistent with quality improvement.

Assisting in developing and maintaining departmental policies and procedures to ensure that the Standards for Pharmacists: Medication Order Review; Dispensing and Compounding; Dialogue and Documentation are met.

Assisting in developing safe written procedures for handling drugs in the hospital.

Assisting in developing and maintaining an up-to-date drug list or Formulary which is approved for use within the hospital by the Medical Advisory Committee.

Assisting in maintaining an adequate library of books and materials of reference for the practice of pharmacy; and to provide drug information support to hospital staff.

Ensure adequate orientation training to the staff, providing opportunity for staff development and optimizing utilization to staff.

Assigns workload and oversees the day to day activity of the Pharmacy Department in the absence of the Manager of Pharmacy.

Practices pharmacy in accordance to the Standards of Practice and relevant legislation such as narcotic and controlled drug acts.
