The Physiotherapist (PT) is an integral member of the Physiotherapy service, Allied Health and clinical programs in which care is provided. Through the provision of patient and non-patient care activities, the PT enhances the quality of life of patients by assessing, planning and implementing individually designed programs of physical treatment to maintain, improve or restore physical functioning and cardio-respiratory health, alleviate pain and prevent physical dysfunction in patients. Principal responsibilities include: providing assessment, intervention, consultation and education services for patients in Stroke Program; collaborating with inter-professional team in providing patient focused care; communicating effectively with patients, families, colleagues and external stakeholders; developing and implementing creative comprehensive patient-centred care plans based on evidence and Stroke Best Practice standards; consulting with internal and external stakeholders regularly to facilitate smooth and timely patient transitions; facilitating and leading patient groups, as appropriate; participating actively in the planning, implementation and evaluation of the program; participating in professional and organizational initiatives supporting best practice, quality management and research; providing education to students, as appropriate.
