Salary: $93,070 - $116,337 per annum (To commensurate with experience and consistent with UHN compensation policy)
Status: Permanent full time (contingent on continued funding)

Position Summary:

The Project Manager position reports to the Senior Program Manager of Connected Care, Clinical Operations, and will be responsible for leading the implementation of a variety of activities such as vaccines, chronic disease management (COPD/CHF/Diabetes), and any new funded opportunities to advance the care for our patients and community members. We are looking for a talented and passionate individual to work alongside the Connected Care, Clinical Operations team as we expand and sustain clinical models of care.

We work closely with the integrated care team aiming at improving the patient, caregiver and care provider experience, with a focus on a creating a seamless care experience for the patient across the health continuum. We collaborate with healthcare providers and system administrators from partner organizations to improve patient transitions from UHN into the community. Our goal is to improve patient satisfaction and their overall health outcomes by following the patient throughout their journey, including wherever possible providing needed care at home.

This role will have a breadth of experience in delivering transformative care solutions to a variety of stakeholders, and is able to lead teams through all phases of the project lifecycle. The Project Manager should have strong change management and facilitation skills, navigate ambiguity with ease, and finds clarity for the team.

Duties:

  • Manage assigned project staff, including allocation of staff resources, delegating, scheduling and prioritizing of staff workloads and developing, mentoring and coaching others.
  • Work with multi-stakeholder groups to identify and/or innovative business problems with available technology including hardware, software, databases and peripherals
  • Handle unusually complex problems and provides highly innovative and creative solutions that will achieve quality and work process improvements
  • Stay up to date on industry news in order to support the development of business case and new project proposals
  • Manage risks by balancing client expectations, project scope and taking early action to resolve problems
  • Assume project ownership of highly complex large scope projects throughout project life cycle
  • Successfully plan, implement and deliver projects within an inter-disciplinary team environment
  • Regularly engage with project sponsors and stakeholders to collect requirements and to ensure success through project lifecycle
  • Apply change management and communication strategies to multiple stakeholders and organizations throughout projects
  • Establish detailed project plan to articulate time, budget, resources, scope and risks in projects
  • Manage project activities related to scope, schedule, resources, communications, risk, financial budget and development of resulting project documentation

Qualifications

  • 3-5years project management experience or equivalent is required;
  • Experience leading and successfully implementing clinical transformation projects in a healthcare setting and/or complex technical implementations is required;
  • Experience leading other staff and/or teams required including direct and indirect reports;
  • Experience in developing and executing communications, engagement and adoption plans (e.g., stakeholder analysis, communication strategies and tactics, awareness campaigns, gathering user feedback)
  • Experience in formal procurement processes, contract negotiations, and vendor management preferred;
  • Strong presentation, writing, and/or training skills
  • Bachelor's Degree in Nursing preferred, or health background and experience working in clinical areas such as hospital and community;
  • Other related degrees such as Health Informatics, Engineering, Business Administration or related discipline from an accredited institution is required;
  • Project Management Professional (PMP) designation and Change Management Certification preferred
  • Flexibility to work outside normal hours to support go live activities is required.
