Position Summary

Toronto Western Family Health Team (TW FHT)'s program links with the surrounding community to provide primary care to families and community members, with special attention to the needs of the aging population, children and women and patient populations who include: a) underserved, at-risk and high needs patients, c) adolescents and emerging adults and d) patients with mental health and addictions.

With the teamwork of physicians, nurse practitioners, primary care nurses, social workers, pharmacists, physiotherapists, occupational therapists, respiratory therapists and chiropodists.

Duties

• Provide consultation and independently screens patients for nutritional intervention, collects and interprets patient health data for in-depth assessment, completes ongoing monitoring and reassessment in collaboration with patients, caregivers, and the health care team
• Through patient-centered approaches, establish priorities, strategies and interventions for individualized treatment plans that allow the patient to achieve optimal outcomes
• Foster a culture of safety by minimizing patient harm, promoting the well-being of patients and health care providers, reducing the likelihood of adverse events, and communicating safety concerns
• Contribute to health education through the development of content and facilitation of 1:1 sessions and educational groups.
• Additional responsibilities may include:
  • Mentoring dietetic students
  • Implementing pertinent medical directives
  • Completing administrative responsibilities
  • Conducting practice-based research

• At minimum, completion of a Bachelor degree in a 4 year Dietitians of Canada (DC) accredited university food/nutrition program degree/discipline or recognized equivalent
• Successful completion of either the Dietitians of Canada accredited 40-52 week Comprehensive, Post-Graduate Dietetic Studentship Program or combined Masters/Internship Practicum Program or Integrated/Stage/Co-ordinated Undergraduate Program or Ryerson Internationally Educated Dietitians Pre-registration Program
• Registration with the College of Dietitians of Ontario
• Membership with Dietitians of Canada, preferred
• Previous experience with Acute General Internal Medicine, Neuro, and Surgical patient population, preferred
• At minimum, 1 year practical and related experience in enteral nutrition support
• Comply with the Code of Ethics and Rules of Conduct according to College of Dietitians of Ontario
• Ability to learn and perform applicable advanced nutrition assessment including Subjective Global Assessment
• Excellent communication, organizational, time management, and problem solving skills
