Wage range: $39.07 - $56.00 per hour (in accordance with collective agreement requirements)
Status: Permanent Full-Time.
Shifts: Days/Evenings/Weekends

Position Summary

As a member of the Combined Surgical Unit's (CSU) Nursing/Multi-disciplinary team, the Registered Nurse Navigator practices in accordance with relevant legislation/standards, meeting the legal and ethical requirements of the profession.

The Combined Surgical Unit is an inpatient care unit that provides care to patients admitted under the following services: General Surgery, Urology, Hand/Plastics, Ophthalmology, Short Stay Orthopaedics and Bariatric Surgery. The majority of General Surgery patients are undergoing surgical treatment for cancer of the GI tract. Some acute non-cancerous conditions exist for patients within this population. Urology patients suffer from urological dysfunction, cancer or the urinary system, or trauma to the system. Hand/plastic patients are split into two categories: reconstructive/microsurgery and acute trauma. Ophthalmology patients are admitted with severe endopthalmitis (eye infections), retinal detachments and glaucoma. Orthopaedic patients are admitted because of sports injuries or arthritic conditions. Bariatric patients will have undergone bypass surgery.

The Registered Nurse Navigator will support patients as they manage their health and navigate the healthcare system in a variety of settings along the health care continuum including – in the community, other healthcare organizations, as well as in home appointments. A core component of the nurse navigator role is collaboration in post-operative assessment and estimated discharge determination for our surgical populations. The Nurse Navigator has a fundamental inpatient role in post-operative clinical education and discharge planning. The Registered Nurse Navigator will collaborate with an inter-disciplinary team, co-develop and provide person-centered support for individuals recovering from surgery and families living with chronic disease. This role maintains a specific focus on discharge education and health care navigation while in the community.

Duties

  • Establishes partnerships with healthcare professionals across settings to extend health promoting and health coaching activities.
  • Assists with system navigation for access to healthcare services/community resources for persons/families living with chronic disease.
  • Provides holistic care to persons and their family members by following their lead.
  • Works collaboratively and in relationship with the inter-professional team while role modelling relationship centered care.
  • Uses concepts from complexity thinking and relational inquiry when in relationship with others.
  • Coordinates care delivery and the development of individualized care plans with the persons internal and external stakeholders including liaising with outside agencies to facilitate seamless care for persons across sectors – primary, acute and community care.
  • Promote RN Health Coaching to healthcare professionals, clients and the community.
  • Advocate for, and contribute to, the establishment of organizational structures and resources to support the growth of the RN Health Coach role in keeping with the direction and priorities of the organization.
  • Serves as an organizational point-of-contact for RN Health Coach role.
  • Working knowledge of community resources for persons living with chronic disease in the Toronto community.
  • Advocate to identify gaps in services, needed for individualized program consideration and system level changes required to meet changing needs of the patient population.
  • Demonstrated leadership skills with strong organizational and self-management skills, and with demonstrated ability to achieve personal, professional and organizational performance targets.
  • Innovative problem solver with demonstrated advanced judgement and decision-making skills.
  • Participates in internal and external committees/working groups

Qualifications

  • Graduate from an undergraduate nursing program; BScN preferred.
  • Basic Cardiac Life Support (B.C.L.S.) certification required. • Registration with the College of Nurses of Ontario.
  • Experience in caring for persons living with chronic disease preferred. • Established in collaborative, self-directed patient-centred nursing practice.
  • Committed to practicing nursing along the healthcare continuum.
  • Familiarized with relationship-centred care based on individual/family needs, concerns, and issues.
  • Demonstrated leadership as innovator and advocate.
  • Demonstrated strong inter-personal skills, written and oral. • Committed to establishing therapeutic relationships with clients and their families.
  • Interested in advancing social justice, cultural competence, health promotion.
  • Knowledge of chronic disease, self-management strategies, and multi morbidity .
  • Ability to function as an independent practitioner and a highly effective team member in a busy multidisciplinary environment.
  • Superior interpersonal, communication and relationship building skills.
  • Effective time management and ability to prioritize competing demands.
  • Ability to work co-operatively and constructively with other team members and develop effective partnerships with community agencies/facilities.
  • Computer proficient
