Registered Practical Nurse Reactivation Care Unit (Job ID: 8122)
St. Joseph's Health Centre, Unity Health Toronto, is proud to announce the opening of a new 21-bed Reactivation Care Unit located on 4G. This unit is dedicated to providing collaborative care for adult and geriatric patients who require therapy, reactivation, and transition planning to successfully transfer to their community or other supportive environments.
The Reactivation Care Unit focuses on patients who no longer need acute care but still require restorative care to aid their transition home or to other settings. As a member of our dedicated interprofessional team, you will deliver patient-centered care based on best practice principles. Our team will work closely with patients, families, and caregivers to provide reactivation therapy aimed at enhancing and optimizing patients' functional abilities and engagement in daily routines.
Note: This unit is not operable until March 2025 (subject to change).
Position Profile:
The Registered Practical Nurse (RPN) will provide high quality nursing care for a variety of medicine patients within an Interprofessional Collaborative Practice philosophy. Being part of this team means being part of an exciting new initiative to help us support patients along their continuum of care and to shape how care is delivered. The RPN works within a changing environment and in inter-professional teams to advocate for and promote wellness while ensuring the delivery of safe, effective and ethical nursing care. The RPN shares his/her knowledge and expertise with others and contributes to the advancement of nursing practice excellence. The RPN continually enhances his/her practice and competencies through ongoing learning, education, experience and reflective practice.
Qualifications:
- Currently registered in good standing as a Registered Practical Nurse with the College of Nurses of Ontario.
- Successful completion of a Practical Nursing Diploma Program or equivalent.
- Current BLS /CPR certificate required.
- Recent nursing experience required. Experience in a rehab, complex continuing care or long-term care facility preferred.
- Demonstrated understanding and commitment to patient and family-centred care.
- Demonstrated ability to organize patient care demands and to set priorities.
- Gentle Persuasive Approach (GPA) training or equivalent is a strong asset.
- Excellent computer skills and competence at using an electronic patient record and other platforms for care delivery.
- Knowledge and experience in falls and pressure ulcer prevention.
- Good communication and interpersonal skills and the ability to function as an interprofessional team player.
- Strong critical thinking/problem-solving skills.
- Satisfactory attendance record.
