Job Description

Candidates are invited to apply for the above positions to work at Villa Colombo Toronto, a 391-bed long-term care facility, located in Toronto, Ontario.

ROLE

In collaboration with the RN, the RPN uses the nursing process to coordinate the direct care activities for assigned resident home areas.

PRINCIPLE RESPONSIBILITIES

  • In the leadership role, promotes effective communication between all team members and ensures Nursing Manager and/or Charge Nurse receives pertinent information in a timely manner.
  • Accountable to the Registered Nursing Unit and/or Charge Nurse for the quality of residents care and effective running of the unit(s) as assigned.
  • Ensures resident care is provided in accordance with established nursing standards and principles, physician's orders, FLTCA and its regulations and Villa Colombo policies.
  • Observes reports and records vital signs, symptoms and conditions of residents and initiates remedial health measures where appropriate.
  • Understands, respects and complies with Resident Bill of Rights.
  • Leads and coordinates the interdisciplinary team in the provision of resident care from shift to shift.
  • Assesses plans, implements and evaluates the individualized plan of care for the residents that ensure a resident and family focus.
  • Administers medications and treatments within their scope of practice. Observes and documents changes in condition resulting from medications/treatments.
  • Makes regular rounds in order to assess each resident and maintain full and constant knowledge of each resident's condition, activities and changing needs.
  • Supervises and assists residents with eating in dining areas at meal times.
  • Responds to resident call bells and assists with ADL's as required.
  • Accountable for documentation, assessments and care plan and ensures that the resident health record (electronic and manual) is complete and accurate including nursing observations and procedures performed, including resident response.
  • Receives medication orders, stores and monitors the safe keeping of drugs and narcotics
  • Conducts physical rounds and liaises with medical staff.
  • In collaboration with the RN, leads coordinates and implements the resident/family conferences.
  • Consults with the Registered Nurse any time there is a change in the resident's health status or behaviour and in complex care and other issues.
  • Sets quarterly and annual objectives for units in collaboration with the Manager, Nursing Unit and the Care Team.
  • Inspires, motivates and coaches direct care staff to meet the individualized needs and expectations of the residents reflecting the mission, vision and values of the organization.
  • Cooperates and maintains professional relationship with staff, residents, families and visitors.
  • Organizes and conducts weekly staff meetings to discuss resident care with team and adjusts plan of care accordingly.
  • Mentors and trains new staff.
  • Assumes with the completion of and/or provides input on probationary and annual performance appraisals for PSWs and other staff as may be required.
  • Assists staff to identify learning needs and to participate in reflective practice.
  • Assists in maintaining a neat, clean and safe environment for residents, staff and all visitors to the home.
  • Participates in staff meetings, in-service and continuing care education programs.
  • Recognizes safety hazards and takes appropriate action to remove/remediate the hazard. Adheres to established facility policies and procedures including safety rules and safe working practices.
  • Completes Employee Incident Reports, offers modified work, completes the investigation and provides First Aid as required.
  • Completes WorxHub for maintenance and relevant health & safety related issues.
  • Ensures open and accurate information is communicated during shift report.
  • Intervenes as required to assist in problem solving and address any resident/family concerns.
  • Complies with Fixing Long Term Care Homes Act and its regulations, and College of Nurses of Ontario Standards of Practice, Villa Colombo policies and procedures and other relevant regulatory and legislative standards.
  • Leads committees and quality projects as assigned

  • Demonstrates skill in managing aggressive behaviour using a resident-centered approach, utilizing GPA and/or PIECES training techniques.
  • Ability to speak Italian an asset

EXPERIENCE & EDUCATION

  • Current certificate of competence from the College of Nurses of Ontario.
  • Current Medication Certificate which meets the College of Nurses Standards of Practice.
  • Minimum of one-year experience in geriatric, rehabilitation or long-term care preferred.
  • Ability to team build, lead, and demonstrate flexibility in a changing health care environment.
  • Certification in Gerontology and current membership in RPNAO is desirable.
  • Ability to speak Italian is an asset.

A police reference check for the vulnerable sector, not older than 6 months, will be required upon hire.
