As an integral member of the multidisciplinary health care team(s), the Rehabilitation Service Coordinator is responsible for the overall patient flow of the program inclusive of planning, organizing, directing, coordinating, and evaluating activities related to patient admission and discharge. In addition, this role provides administrative and clinical leadership to all staff in a supportive, empowering manner while facilitating the effective and efficient admission and discharge of patients. Key responsibilities include: leading and/or participating in developing and implementing organizational initiatives regarding quality improvements and program development; planning, organizing, directing, coordinating, and evaluating activities related to patient admission and discharge, ensuring efficient and effective program operation by providing mentorship and promoting a supportive learning environment for health professions and administrative staff; acting as a clinical resource for the interprofessional team; establishing and maintaining a systematic review and assessment of department capacity. Service Coordinators develop and maintain strong relationships with patients/families, external partners, and other stakeholders.
