Salary: $36.38 - $45.48 per hour (To commensurate with experience and consistent with UHN compensation policy)
Status: Temporary Full-Time ( one year )

University Health Network (UHN) is looking for an experienced professional to fill the key role of School and Work Transitions Program Counsellor (POGO and AYA) at Princess Margaret Hospital.

The POGO School and Work Transitions Program is a specialized counselling service that supports survivors of childhood cancer and brain tumours who are experiencing difficulties transitioning to school and/or work as a direct result of their disease or treatment. The POGO Transitions Program's goal is to facilitate a smoother transition from secondary school or from being off school or work to further education or employment.

The AYA Program provides personalized, supportive care related to concerns common to adolescent and young adults with cancer aged 39 and under. These concerns can include fertility, sexual health, body image, mental health, school and work, peer connection, family support and wellness. The goal of the school & work counsellor is to support with resumes, accommodations, scholarship applications, volunteer resources and any education/employment concerns.

DUTIES:

  • Provide one-to-one vocational and academic counselling and advisement to eligible clients/patients and identify education and employment barriers
  • Develop client-specific strategies to successfully achieve transition to further education or employment
  • Attend/establish links with secondary and post-secondary schools, places of employment and community agencies as necessary to support clients
  • Participate in educational and informational program presentations to various audiences and actively engage in, identify and promote the POGO School and Work Transitions program initiatives, including the Survivor to Survivor (S2S) Network
  • Supports with AYA in person/virtual events and employment/education webinar facilitation
  • Ability to interpret neuropsychological/psychoeducational reports as applicable
  • Exchange knowledge with health care providers, educators and counsellors (the ongoing resources for individual clients)
  • Provide information and resources and advocate for survivors and parents
  • Track service delivery volumes and report to POGO/AYA
  • Provide updates for the POGO School and Work Transitions Program and PM AYA webpages
  • Travel within GTA is required
  • Works in compliance of the Occupational Health & Safety Act and its regulations, reporting hazards, deficiencies and contravention's of the Act, in a timely manner.

Qualifications

QUALIFICATIONS:

  • Completion of a bachelors degree in Education, Psychology, Disability Studies or recognized equivalent
  • Post graduate certificate in career counselling/development an asset
  • Minimum (3-4) years practical and related experience required
  • Counselling experience in a healthcare or education setting
  • Experience providing academic advisement an asset
  • Experience with job preparation, including resume and cover letter writing an asset
  • Experience working with a diverse client population
  • Experience with complex case management, outreach and referrals
  • Valid driver's license and access to vehicle preferred
