Social workers in health care provide in-depth bio-psychosocial assessments, diagnostic, treatment and evaluation services for individual, family group and couples. Social workers provide individual, group and family counselling, conduct discharge planning, resource referral matching, consultation and collaboration, teaching, research and patient/family education.

Unity Health Toronto's Inpatient Mental Health Program located at the St. Michael's site offers short-term hospital admissions for adults with mental disorders who would benefit from symptom stabilization, medication reassessment, individual and group counselling, psychoeducation and coordination of continuing care resources for continuing care treatment and community support after discharge. Inpatient services are delivered by a highly skilled interprofessional team consisting of psychiatrists, registered nurses, registered practical nurses, occupational therapists, social workers, pharmacists and peer support specialists.

The successful candidate for this position must demonstrate the ability to:

  • Work effectively as a member of an inter-professional team, collaborating with other disciplines to provide comprehensive patient care
  • Actively contribute to the academic mandate of the organization as an educator, researcher, resource or support for staff, students and members of the academic and professional community
  • Identify and address educational needs for patients and their families
  • Demonstrate a commitment to ongoing professional development in order to continue to provide optimal patient care
  • Commit to, and participates in, ongoing quality improvement in the organization
  • Provide patient care in accordance with all professional and organizational standards
  • Demonstrate an understanding of the social determinants of health and knowledge of how to access and link patients to appropriate community resources and services such as: OW, ODSP, Trillium, Community Housing, CCAC etc.
  • Demonstrate excellent knowledge of recovery-oriented and trauma informed mental health care delivery in an inpatient mental health context

QUALIFICATIONS:

  • Must have a Master's Degree in Social Work (MSW)
  • Must be currently registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) for practice in all applicable clinical areas
  • Eligible to engage in student education with academic partners
  • Current CPR certification
  • Sound clinical acumen and experience working with patients and families experiencing a mental health crisis required
  • Experience conducting indepth psychosocial assessments and recommending appropriate interventions to help patients achieve optimum recovery and quality of life
  • Experience facilitating short-term individual and group counselling (CBT, DBT, Trauma, Motivational interviewing, MBCT)
  • Experience facilitating interprofessional meetings with families
  • Experience identifying appropriate continuing care supports for patients after discharge
  • Demonstrated ability to organize and coordinate collaborative care plan development with internal and external partners
  • Superior interpersonal skills with the ability to collaborate as a member of an interprofessional team but also work autonomously
  • Excellent therapeutic communication and counselling skills
  • Proven flexibility and adaptability in a fast-paced environment
  • Excellent organizational, analytical, negotiation, time management and problem solving skills
  • Proficiency in the use of computers with proven experience in data interpretation and extraction of information;
  • Demonstrated commitment to professional learning;

Must be willing to work evenings and/or weekends as directed by the Inpatient Mental Health Program
