The Toronto Grace Health Centre (TGHC) provides, specialized care to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Transitional care, and Palliative Care services. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 3 sites – the Main site (Church & Bloor St) and our Harbour Lights site and Specialized Care Centre (Victoria Park & Mc Nicoll). POSITION SUMMARY

The Patient Care Department at the Salvation Army, Toronto Grace Health Centre is currently seeking a Social Worker. The Social Worker is a member of the interprofessional team provides safe, quality, patient centered care to individuals with chronic and terminal illnesses including providing support to their families and significant others. The Social Worker maintains a link between the patient, family and community to support the primary social worker and interprofessional care team’s individual patient care plan.

MAIN RESPONSIBILTIES

Provide social work coverage and support services to patients/families on differing units depending on current need. This would include rehab, complex continuing care and palliative care programs.

Provide a range of social work services including psychosocial assessment, crisis intervention, discharge planning and counseling to patients and their families.

Act as liaison and assist patients and staff with community resources.

Provide comprehensive discharge planning services within Post-Acute Care Rehabilitation and the Complex Continuing Care.

Provide in-depth transfer of accountability reports to ensure a seamless transition of patient and family care.

QUALIFICATIONS AND EDUCATION:

Master’s degree or working towards a Master Degree in Social Work from an accredited School of Social

Work, or equivalent degree as recognized by the Ontario College of Social Worker is required.

Flexible schedule is required.

Experience in a hospital or health care setting.

Experience with discharge planning is an asset and preferred.

Knowledge of the acute, sub-acute and community sector. Knowledge of community resources an asset.

Demonstrate ability to welcome change and continuous improvement.

Experience in mental health is asset.

Strong communicator with proven problem solving and interdisciplinary skills.

Member in good standing with the OCSWSSW.

Knowledgeable of cultural system perspective that includes family, community, religious and spiritual resources.

Excellent interpersonal skills as demonstrated by your ability to develop and maintain positive relationships with patients/families, and members of the health care team.

Demonstrated facilitation and problem solving skills.

Exceptional time management skills.

Excellent communication skills (oral and written) facilitating effective communication in case reporting and assessments, with patients, families, other healthcare team members and volunteers.

Self-motivated, team player, and ability to thrive in a fast-paced environment.

High level of integrity, leads by example and exemplifies our Core Organizational Values.

Excellent attendance record.

The successful candidate must provide an acceptable Police Records Check (Vulnerable Sector Screening).
