Job Description

Reports to: Manager, Allied Health
Hours: Various
Wage range: $42.21 per hour - $52.76 per hour (To commensurate with experience and consistent with UHN compensation policy )
Status: Temporary Part-Time ( November 2026 )
Closing Date: January 18, 2025

In the provision of patient and non-patient care activities, the primary responsibilities of the SOCIAL WORKER in the inpatient Geriatric, MSK, and Multi-System Rehab include:

  • The innovative application of social work processes in psychosocial assessment, education, supportive and adjustment counselling with patients and families;
  • The application of knowledge of the rehabilitation continuum, philosophy and process environment for patients;
  • Conducting assessments independently and as part of an interprofessional team; coordination of care plans and future care planning, including appropriate referral to community agencies and support services;
  • Collaboration with the interprofessional team and community-based service providers; providing leadership related to team communication with patients and families;
  • Participation in professional, program and hospital-wide program development and meetings;
  • Participation in teaching initiatives including student education; involvement in quality management and research initiatives;
  • Compliance with and commitment to patient safety responsibilities and corresponding hospital plans, policies and procedures in order to ensure a safe environment for patients.

Qualifications

  • Completion of a Master's degree in Social Work (M.S.W.) or equivalent
  • Registration with Ontario College of Certified Social Workers and Social Service Workers (OCCSWSSW)
  • Membership with Ontario Association of Social Workers (OASW) preferred
  • Two years experience working with a multi-disciplinary team in a health care setting preferred Outpatient &/or Inpatient setting.
  • Experience working with adults with complex medical needs preferred
  • Knowledge regarding community resources available to the gerontological population
  • Excellent interpersonal skills, time management and organizational skills
  • Excellent verbal and written communication skills
  • Excellent problem solving skills
  • Research experience/education an asset
  • Student education/supervision an asset
  • Knowledge of policies, legislation, programs and issues related to social welfare and healthcare institutions and services
  • Computer literacy preferred
