Speech Language Pathologist Reactivation Care Unit (Job ID: 8138)
St. Joseph's Health Centre, Unity Health Toronto, is proud to announce the opening of a new 21-bed Reactivation Care Unit located on 4G. This unit is dedicated to providing collaborative care for adult and geriatric patients who require therapy, reactivation, and transition planning to successfully transfer to their community or other supportive environments.
The Reactivation Care Unit focuses on patients who no longer need acute care but still require restorative care to aid their transition home or to other settings. As a member of our dedicated interprofessional team, you will deliver patient-centered care based on best practice principles. Our team will work closely with patients, families, and caregivers to provide reactivation therapy aimed at enhancing and optimizing patients' functional abilities and engagement in daily routines.
Note: This unit is not operable until March 2025 (subject to change).
Job Profile:
Speech-Language Pathologists (SLPs) provide screening, assessment, management, education, counseling and assistance with discharge planning for patients with swallowing, speech, language, voice, and cognitive-communication impairments at Unity Health Toronto. The purpose of the speech-language pathology profession is to identify, assess, treat and prevent communication and swallowing dysfunction or disorders, while providing an inclusive, patient-centred intervention. Functional outcomes may include improvements in quality of life, health, self-esteem, confidence, vocational/educational success, and family/workplace/community participation. SLPs may also contribute to program development, quality and practice-based research.
The Speech-Language Pathologist (SLP) is a self-regulated professional who:
- Uses appropriate tools and methods to gather relevant objective and subjective information related to the factors that impact the patient's swallowing and communication.
- Collaborates with the patient*, their family**, and the interprofessional team to make recommendations for appropriate interventions, service frequency, service duration, and monitoring
- Delivers care based on evidence and relevant best-practices
- Facilitates early discharge planning with immediate and appropriate referral(s) to community services to decrease length of stay and/or to facilitate continuity of care and/or reintegration into community.
- Supervises additional roles as relevant (e.g. SLP students and Support Personnel)
- Supports the organization's academic mandate by participating in student learning through supervision, evaluation, teaching etc.
- Contributes to intraprofessional and interprofessional education for internal and/or external stakeholders
- Participates in clinical program development, research, and quality assurance activities
Qualifications
- Must have a Master's degree in Speech-Language Pathology (or equivalent)
- Must have current BLS training
- Knowledge, skills and judgement to safely complete the job requirements
- Strong oral and written communication skills
- Strong organizational and time management skills
- Must be able to work co-operatively and constructively with other interprofessional team members;
- Demonstrated flexibility in provision of patient care
