Unit Clerk Reactivation Care Unit (Job ID: 8166)
St. Joseph's Health Centre, Unity Health Toronto, is proud to announce the opening of a new 21-bed Reactivation Care Unit located on 4G. This unit is dedicated to providing collaborative care for adult and geriatric patients who require therapy, reactivation, and transition planning to successfully transfer to their community or other supportive environments.
The Reactivation Care Unit focuses on patients who no longer need acute care but still require restorative care to aid their transition home or to other settings. As a member of our dedicated interprofessional team, you will deliver patient-centered care based on best practice principles. Our team will work closely with patients, families, and caregivers to provide reactivation therapy aimed at enhancing and optimizing patients' functional abilities and engagement in daily routines.
Note: This unit is not operable until March 2025 (subject to change).
Job Profile:
The Unit Clerk is highly-adaptable, highly-flexible, sweats the details, and effectively communicates with patients and families, the different areas within the Health Centre and with external partners as required. Principle responsibilities include: supporting the interprofessional team; supporting continuous quality improvement and innovation; typing, filing, photocopying, faxing, ordering and maintaining office supplies; filing of appropriate information in the IRT¿s data management system as per protocol; booking and coordinating appointments accurately and efficiently; compiling and entering program service statistics and data; maintaining an accurate and confidential filing system; preparing reports including letters, memos, agendas, meeting minutes; organizing room bookings, conference/meeting requests and catering; tracking and submitting payroll accurately; scheduling staff duties as assigned; providing administrative support to Patient Care Manager(s) as required; providing coverage and assistance to other program administrative positions as required; understanding of, compliance with, and commitment to, patient safety responsibilities and corresponding Health Centre plans, policies and procedures in order to ensure a safe environment for our patients and families, and our customers/partners. Responsibilities include, but not limited to:
- Manage daily staff scheduling/roster
- Prepare, post, and maintain staffing schedules.
- Process payroll for staff, ensuring accuracy and timely submission
- Order supplies and handle filing.
- Perform other clerical tasks as assigned.
- Provide clerical support to registered staff, residential areas/communities, and Managers.
- Collect and distribute mail.
- Schedule patient care conferences and appointments.
- Arrange transportation for patients as needed.
- Collaborate and communicate with other departments as required.
- Support 6G Transitional Care Unit on weekends as appropriate
Qualifications include:
- Minimum Grade 12 education.
- Recent experience as a Unit Clerk in hospital experience or other clerical experience.
- Excellent English communication skills, both verbal and written.
- Basic mathematical/numerical skills and experience ordering supplies.
- Polite & efficient telephone manner.
- Knowledge of medical terminology required.
- Computer experience required, Intermediate level skills in MS Word and Excel.
- Proven ability to work independently and show initiative.
- Demonstrated ability to prioritize.
- Able to work well under pressure & co-operate with all Health Centre staff and patients.
- Satisfactory attendance record.
