RESPONSIBILITIES:
• Program planning and development with management, Ontario Health, regional partners, and local community partners in Oxford County
• Work closely with local Oxford community agencies for seamless integration and delivery of services
• Lead and participate in various working groups related to the development of the Oxford Hart Hub.
• Develop and maintain constructive working relationships with community agencies, primary care, and other professionals having contact with Mental Health Services. The Coordinator will encourage positive public relations, co-ordination of services and information sharing, and also participate on hospital and community committees.
• Provide oversight to demands of program such as patient flow, gaps in service, service efficacy and triage
• Oversee implementation of new clinical models of service and integration with medical management models
• Receive regularly scheduled clinical and administrative supervision by the Director of Mental Health Services or delegate.
• Provide electronic assessments, complete progress notes and termination summaries for patient files, as needed.
• Participate in case conferences, team meetings, and Mental Health departmental meetings, to ensure the co-ordination and planning of patient programs and services with the centralized intake service.
• Participate in clinical and administrative unit meetings, social work meetings, and other meetings necessary for the effective performance as a social worker within the hospital.
• Keep abreast of new developments within the field of social work and related fields. Maintain and upgrade skills as required.
• Act as Field Practicum instructor to Social Work students placed at the hospital, as requested by the Director of Mental Health Services.
• Carry out a teaching role within Mental Health Services through participation, and presentation at the regularly scheduled staff education seminars and other educational forums existing within the hospital. This role may also extend to the community.
• Maintain timely and accurate statistical records of activities for Departmental, Hospital, and Ministry purposes.
• Consult with Director of Mental Health Services, when administrative, therapeutic, or legal matters could result in significant or serious outcome to patient, hospital, or therapist.
• Carry out other duties as required by the Director of Mental Health Services.

• Superior written and verbal communication skills are mandatory
• Excellent ability to maintain, monitor and report on all data relevant to centralized intake processes for the department and region.
• Demonstrated ability to deal congenially with a variety of people efficiently and effectively with minimal supervision
• Organizational skill and demonstrated ability to handle and respect confidential information with fact and diplomacy are essential.
QUALIFICATIONS:
EDUCATION
• Master's degree in Social Work from an accredited university
• Leadership and Management Certificate or Training is not mandatory but would be considered an asset.
REGISTRATION
• Must be a member in good standing with the Ontario College of Social Workers and Social Service Workers
