Job Description

Site: West Park Healthcare Centre - 170 Emmett Ave.
Department: Provincial Long-Term Ventilation Strategy
Reports to: Manager, Program Services
Hours: 37.5 hours per week
Salary: To commensurate with experience and consistent with UHN compensation policy
Status: Temporary, Full-Time (approximately 12 months)
Closing Date: January 14, 2025

Position Summary

The primary purpose of this role is to coordinate and facilitate safe and timely transitions of Long-Term Ventilated (LTV) patients from acute care organizations to alternate locations utilizing the resources of the Long-Term Ventilation Strategy in collaboration with external partners and stake holders.

This position may include travel within Ontario.

Clinical Lead for Toronto Region LTV Centralized Waitlist

  • Work directly with OH Toronto Region and Critical Care table to decant patients in Toronto Region ICUs to LTV sites (Michael Garron, West Park and Toronto Grace)
  • Lead regular huddles with the three LTV Sites to pull patients according to bed availability
  • Work closely with ICUs and the clinical teams to identify appropriate LTV patients, offer support or clinical advice
  • Manage the centralized LTV waitlist and forward facing list
  • Reporting for Provincial Long-Term Ventilation Strategy Indicators

LTV Strategy Co-ordination (Pull Strategy, Transition Program)

  • Regular visits to Greater Toronto Area ICUs and conduct all initial pre-assessments
  • Work with ICU patients, families and medical teams to look at possible discharge options

Long-Term Ventilation Centre of Excellence Activities

  • Oversee coordination of external trainings for LTV patients going or living in the community
  • Member of Community Vent Team
    Perform respiratory clinical assessments in community as required (assessments, education, trach changes etc.)
  • Participates in weekly Community Vent team rounds and collaborates with the interdisciplinary team as required to ensure timely and appropriate changes to patient care plans as required.
  • Provide written and verbal progress reports as required
  • Ensure the collection/analysis of COE data is completed efficiently and effectively in support of achieving the COE objectives.

Long-Term Ventilation In-Patient Unit

  • Serve as the coordinating contact between patients, family, team members and referring agencies for LTV/THV.
  • Liaise with the Clinical Manager Provincial LTV to review/update the LTV/THV wait list for West Park.
  • Coordinate timely assessments and admission of appropriate referrals to LTV/THV service and liaise with physicians and the inter-professional team.
  • Co-ordinate the ongoing care of patients from pre-admission to post-discharge while meeting length of stay, occupancy rate and accountability targets.
  • Demonstrated exceptional clinical assessment skills including the ability to assess clients from pre-admission to post-discharge
  • Knowledge of external agencies and resources
  • Demonstrated ability to partner and work effectively with staff, the interprofessional team, physicians, patients, families and external agencies
  • Demonstrated ability to address complex clinical issues within the context of social determinants of health
  • Ability to maintain effectiveness in varying environments and with different tasks responsibilities and people
  • Demonstrated knowledge of and ability to provide person-centered care

  • Identification and prevention of safety issues and problems
  • Taking corrective action, where possible
  • Attendance at safety meetings/training/updates in accordance with the Centre's standards
  • Perform other duties as assigned

Qualifications

Required (Minimum):

  • Respiratory Therapy Diploma in conjunction with a Bachelor's of Science
  • Masters in a related field preferred
  • Registration with the College of Respiratory Therapists of Ontario (in good standing)
  • Five years within the practice profession; with LTV experience preferred
  • 3 years of Clinical leadership experience preferred
  • Basic Cardiac Life Support (C Level) Heart Saver Plus Canada (on employment)
  • Excellent interpersonal and communication skills (verbal & written) required
  • Strong customer service focus and excellent problem-solving skills required
