Job Description:

Reporting to the Primary Health Care Manager, or his/her designate, Primary Health Care Teams and in collaboration with physicians and health care provider team members, the Registered Nurse (Nurse Practitioner) will provide Primary Health Care (PHC) services to assigned populations which may include: urban populations, visiting services to rural communities, and/or priority populations (e.g. newcomers, teens, residents in long term care facilities) as determined by community and program needs.

As the PHC Team evolves, and based on needs of the population, the position may be required to provide new practices such as nurse-led extended hour clinic services. Primary Health Care services include health assessment, health promotion and education, disease and injury prevention and assessment, treatment interventions; collaboration with, and referral to, other health care providers; participation in quality improvement, patient safety, community development, clinical practice redesign, and evaluation.


Experience

  • Three (3) years experience working as a Registered Nurse
  • Experience as RN (NP) in a Primary Health Care setting preferred.

Education

  • Master of Nursing - Nurse Practitioner (MN-NP)

Licenses

  • Valid Class 5 driver's license

Other Education and Training

  • Advanced Cardiac Life Support Certificate (ACLS)
  • Basic Life Support Certificate (BLS)

  • Ability to make decisions on patient care and coordination of equipment
  • Ability to competently use computers and software applications
  • Knowledge in Microsoft Office Suite
  • Knowledge in community development
  • Ability to establish and maintain effective work relationships.

Other Information

  • Travel may be required to provide service to sites within the area
  • Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)
