Job Description: Provides case management services, which includes assessing, planning, monitoring and evaluating for determining the care needs of clients, collaborative coordination of services and case management process.

Education

  • Bachelor degree in Social Work, Psychology or other health related discipline

Licenses

  • Valid Class 5 driver's license
