Job Description:

The Primary Health Care (PHC) Registered Nurse (RN) is an integral member of the primary health care team, providing and coordinating initial, continuing and comprehensive nursing services to patients in the PHC sites. The PHC RN promotes quality health care outcomes working within established standards for case management practice to ensure that patients/clients receive a high standard of primary health care. This will be done in accordance with Saskatchewan Health Authority statement and Primary Health Care policies and procedures of the facility and department. The PHC RN initiates and/or participates in community development initiatives within the site. Accountability will be maintained through a close working relationship with the Primary Health Care Leadership.

Experience

  • Experience in case management
  • Experience in teaching adults, facilitation skills, and leading group activities

Education

  • Bachelor of Science in Nursing (BScN)

Other Education and Training
