Job Description: Reporting to the Home Care Manager the Client Case Coordinator is responsible for the assessment of clients, coordination and evaluation of Home Care services and case management of Region Residents. The Client Case Coordinator will be responsible for long term care placement in the rural area under the Home Care program. The Client Case Coordinator will ensure that the delivery of home care and case management services is consistent with the policies, vision, mission and philosophy of the Saskatchewan Health Authority.

Education

  • Bachelor of Science in Nursing (BScN)

Other Education and Training

  • Practical Nursing diploma with 4 years experience is acceptable
  • Bachelor of Social Work is acceptable
  • Bachelor of Psychiatric Nursing is acceptable
  • Psychiatric Nursing diploma is acceptable
  • Nursing diploma is acceptable
