Job Description: The Assessor Coordinator Senior will assist the client and family to obtain a high quality of care by jointly developing, implementing and evaluating a plan of care that meets the needs of the client and their family/supporters as the client leaves the hospital to continue with care or to support the client to remain independent in the home setting.

  • Bachelor of Science in Nursing (BScN)

Licenses

  • Valid Class 5 driver's license
