Job Description: The Public Health Nurse (PHN) provides services within the Population Health Promotion Model and Primary Health Care framework. The PHN translates knowledge from health and social sciences to support health-enhancing behaviors of individuals, families and population groups through targeted interventions, programs, and advocacy. Inherent in the role are knowledge, skills, and abilities specific to health promotion, prevention, and protection. The PHN works relatively independently and makes decisions based on sound assessments in accordance with Public Health Act, Public Health Nursing program policies and the Health Authority, CRNS Standards of Practice, Canadian Community Health Nursing Standards of Practice, and other relevant legislation. This employee liaises with, and refers to, other health personnel, sectors and community groups. The PHN works within established guidelines established by SHA policy to plan, organize, implement and evaluate nursing programs in an assigned area.

Experience
