Job Description: Reporting to the Primary Health Care (PHC) Regional Director (or Designate such as Community Health Services Manager), PHC Teams and in collaboration with physicians and health care provider team members, the Registered Nurse (Nurse Practitioner) will provide visiting PHC services to sites within the service area. Assigned populations may include: newcomers, teens and, residents in long term care facilities as determined by community and program needs. As the PHC Team evolves, and based on needs of the population, the position may be required to provide new practices such as nurse led extended hour clinic services and/or first on-call support. PHC services include: health assessment, health promotion and education, disease and injury prevention and assessment, treatment interventions; collaboration with, and referral to, other health care providers; participation in Lean, patient safety, community development, clinical practice redesign, quality improvement, and evaluation.

Experience

  • Experience as RN (NP) in a Primary Health Care setting preferred.
  • Three (3) years full-time experience as a Registered Nurse

Education

  • Master of Nursing - Nurse Practitioner (MN-NP)

Licenses

  • Valid Class 5 driver's license

Other Education and Training

  • BLS for Healthcare Provider (Level C) - Heart and Stroke
  • ACLS training required upon hire

  • Intermediate - Leadership skills
  • Intermediate - Interpersonal skills
  • Intermediate - Communication skills to provide/obtain verbal and/or written opinions/advice

Knowledge and Abilities

  • Knowledge in community development
  • Ability to make decisions, which must include coordination of staffing
  • Ability to work independently and as a member of a multi-disciplinary team
  • Ability to establish and maintain effective work relationships.
  • Ability to competently use computers and software applications
  • Knowledge in Microsoft Office Suite
