Salary or Pay Band: Pay Band Nurse Practitioner $52.590 to $62.910

Job Description: Provides primary health care services to the community, which includes health promotion & protection, disease & injury prevention, provision of supportive & instructive treatment and emergency services. Performs health care management, pharmacotherapeutics & therapeutic interventions. Utilize & synthesize multiple sources appropriate health assessment & differential diagnosis of client/patient status. Assess needs for ordering & interpreting laboratory, radiology & ultrasound tests. Works in conjunction with the client, family & health providers to develop & implement holistic treatment, interventions & support. Facilitate case management model of clinical care. Assist individuals, families & communities to increase their knowledge, control & influence of health determinants to maintain & improve their health status. Work with stakeholders to improve the health of at risk youth. Support/assist in the development/implementation of quality initiatives to benefit service delivery models.

Human Resources Exemption: No

Experience

  • Three years fulltime experience as a Registered Nurse
  • One (1) year experience with community development/health promotion activities

Education

  • Bachelor of Science in Nursing (BScN)

Licenses

  • Valid Class 5 driver's license

Other Education and Training

  • Primary Health Care Nurse Practitioner Specialization
  • Master of Nursing is acceptable
  • Basic Cardiac Life Support Level C Certificate (BCLS)
