Job Description: The Mental Health Therapist - Staff is an integral member of the Primary Health Care Team and works in part of the network team in the medical home model. As a member of a multidisciplinary primary health care team, the incumbent is primarily responsible for behavioral health supports for a number of conditions under the care of the Primary Health Care Provider. The services to be delivered through this position include screening functional assessment and develops behavioral change plans in conjunction with the Primary Health Care Provider. The Mental Health Therapist develops and implements a behavioral healthcare plan and makes referrals to as required.

  • Experience with clinical interventions related to Primary Health Care

Education

  • Degree in Human Services/Health Sciences or related field

  • Licensed, and in good standing, with regulatory body, if applicable
