Job Description:

Provides case management services, which includes assessing, planning, monitoring and evaluating for determining the care needs of clients, collaborative coordination of services and case management process.

Travel Required: No

Experience

• Minimum of 1 year recognized Mental Health and/or Addictions Counselling experience is an asset.
• Clinical application of therapeutic approaches and counselling techniques is an asset.
• Experience working with Indigenous and/or other marginalized people in a therapeutic setting is an asset.
• Additional training in human services management or administration is an asset.

Education and Training

• Bachelor degree in Social Work, Psychology, Nursing or Addictions Counselling.
• Registered or eligible for registration with the appropriate licensing body.
• Cultural Sensitivity training is a definite asset.

Other Information

• Registered/eligible with the Saskatchewan Association of Social Workers (SASW)
• Preference to HSAS members as per the SAHO/HSAS Collective Agreement
