Job Description: Under the direction of a Program Manager and as part of the interdisciplinary Community Outreach and Support Team, the Assessor Coordinator provides a range of screening, triage, assessment, treatment and case coordination services to a community mental health and addictions population. The Community Outreach and Support Team provide crisis intervention, support, assessment and follow-up plans to clients in the community who are in crisis or require assistance. The team also assists in the successful stabilization and/or transition of acute patients from hospital/detox to community or alternatively to prevent admission/readmission by providing short term acute mental health/addiction services to those who present in crisis. Follow-up may include brief monitoring/support, referral to appropriate community organizations/support networks or collaboration with existing supportive organizations. Prevention of ongoing or future crisis is a goal of the program. The model of service is mental health and addiction outreach and crisis intervention. Services are provided through extended hours (days, evenings and weekends).

Education

  • Recognized degree/diploma in nursing, social work degree or addiction counsellor degree.
  • Must have a current practicing registration with their professional association.

Experience

  • One year of recognized experience (postgraduate) in working within community mental health and addiction services.

Knowledge, Skills and Abilities

  • Knowledge of a variety of mental health disorders and interventions required to enable clients to sustain stability.
  • Knowledge of a variety of addiction and harm reduction strategies and interventions required to enable clients to sustain stability.
  • Knowledge of community resources is essential.
  • Work experience with mental health and addictions community outreach and competence in delivering mental health and addiction crisis interventions.
  • Demonstrated clinical assessment skills, case planning, and consultation experience.
  • Able to work independently and in a self-motivated manner, ability to respond under pressure.
  • Demonstrated ability to work collaboratively and in partnership with a multidisciplinary team and other agencies in the community.
  • Demonstrated cultural competency skills.
  • Knowledge and practical application of the Mental Health Services Act.
  • Demonstrated consultation and public relation skills with the ability to maintain effective work relationships.
  • Demonstrated interpersonal competence including effective problem solving, decision making and conflict resolution skills.
  • Demonstrated communication skills - both written and oral.
  • Ability to enter and retrieve data in a computer database system

  • Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement. HSAS seniority within the Saskatchewan Health Authority will be considered under the Selection Criteria.
