Job Description: The incumbent is an integral part of the Advance Care Planning Program reporting to the Director, Clinical Standards & Professional Practice (Urban). The primary focus is to implement and coordinate the Advance Care Planning initiative and to research, develop, and facilitate educational events including the development and dissemination of resource materials regarding Advance Care Planning. Education will be provided to members of the health care team across the continuum, contacts in surrounding health regions, and the general public. The incumbent is accountable to the Director of Clinical Standards & Professional Practice and works collaboratively with the Advance Care Planning Steering Committee.

Education:

  • Requires an undergraduate degree in a related health care discipline such as social work, nursing, physical therapy, occupational therapy or a degree in health administration or education
  • Must have licensure and be a member in good standing of the appropriate professional association.

Experience:

  • Must have a minimum of five years experience with end of life care and adult education or, community development and health promotion.

Knowledge, Skills & Abilities:

  • Demonstrates a strong commitment to patient centered care

  • Demonstrates excellent oral and written communication skills
  • Demonstrates the ability to have a meaningful Goals of Care discussion
  • Requires coping skills to work with persons dealing with stress and other end of life issues
  • Demonstrates well developed critical thinking ability and sound judgment
  • Demonstrates ability to do research and apply knowledge related to current best practice
  • Demonstrates knowledge of the principles of lean improvement work
  • Understands principles of community development and health promotion
  • Demonstrates the ability to establish and maintain effective working relationships with a wide variety of public and private agencies
  • Demonstrates initiative, sound judgment, and excellent organizational skills in order to plan and coordinate information gathering from several sources
  • Demonstrates resourcefulness and the ability to work independently, but in collaboration with others as required
