Job Description: Reporting to the Urban Primary Health Care Manager, the Registered Nurse is an integral member of the primary health care team, providing and coordinating initial, continuing and comprehensive nursing services to patients in the PHC sites. This Registered Nurse promotes quality health care outcomes working within established standards for case management practice. The Registered Nurse initiates and/or participates in community development initiatives and participates in the evaluation of Primary Health Care initiatives within the site.

Demonstrated knowledge of chronic disease prevention and management. Demonstrated clinical competency including critical thinking skills and confidence to work autonomously. Demonstrates sound evidence-based knowledge and judgment when making decisions about a client's nursing care. Demonstrates knowledge of population health promotion theory and practice and ability to transfer this knowledge into practice. Knowledge of and sensitivity to the impact of social, economic, environmental and cultural issues on chronic disease. Team leadership and facilitation skills including an understanding of the fundamentals of team dynamics. Demonstrates the ability to manage conflict toward effective outcomes. Effective communicator with active listening and motivational interviewing skills. Demonstrates empathy, trust and respect in the provision of care. Demonstrates ability to accept and implement change. Knowledge and application of quality improvement principles, tools, and outcomes. Strong computer skills. Able to prioritize tasks and work under pressure. Knowledge of the lean management systems. Systems thinking approach to health care delivery.
