Job Description: Under the direction of the Primary Health Care Manager, the Registered Nurse will be responsible for providing nursing services to urban and rural Primary Health Care clients within the Saskatchewan Health Authority (former Regina Qu'Appelle Health Region). This position will be part of Intermediate Care Services (formerly Seniors' House Calls Service) within the Primary Health Care Networks. This service addresses a provincial health system service gap, where some seniors (adults aged 65+) with multiple complex health issues have difficulty in accessing an office based primary care provider. The goals for this service are: to provide the most at risk seniors with home based access to higher level health care providers, including MDs and NPs; to mitigate the risk for worsening health states and acute care episodes leading to ED visits and hospitalizations for at risk seniors; to reduce system pressures on acute care services, particularly EDs and hospitals; to provide cost effective and high quality health care using a patient centered and community based approach. Practitioners in this service are assigned within the Primary Health Care Networks and provide support to the interdisciplinary Primary Health Care team in client care provision not exclusive to the seniors' population.
