Job Description: Under the direction of the Primary Health Care Manager, the Registered Nurse will be responsible for providing nursing services to urban and rural Primary Health Care clients within the Saskatchewan Health Authority (former Regina Qu'Appelle Health Region). This position will be part of Intermediate Care Services (formerly Seniors' House Calls Service) within the Primary Health Care Networks. This service addresses a provincial health system service gap, where some seniors (adults aged 65+) with multiple complex health issues have difficulty in accessing an office based primary care provider. The goals for this service are: to provide the most at risk seniors with home based access to higher level health care providers, including MDs and NPs; to mitigate the risk for worsening health states and acute care episodes leading to ED visits and hospitalizations for at risk seniors; to reduce system pressures on acute care services, particularly EDs and hospitals; to provide cost effective and high quality health care using a patient centered and community based approach. Practitioners in this service are assigned within the Primary Health Care Networks and provide support to the interdisciplinary Primary Health Care team in client care provision not exclusive to the seniors' population.

Education

  • Bachelor of Science in Nursing (BScN)
  • Current BCLS certification

Experience

  • A minimum of two (2) years recent related nursing experience is required.
  • Community nursing experience is an asset.

Knowledge, Skills and Abilities

Ability to make independent nursing judgments based on evidence based practice and former RQHR guidelines. Demonstrates good communication skills with clients, peers,
interdisciplinary team members, physicians and related departments. Demonstrates knowledge of the nursing process. Current clinical knowledge and skill is required in a broad
range of areas. Ability to initiate and exercise critical thinking and decision making. Ability to demonstrate appropriate level of professional conduct and judgment. Must have
the ability to promote effective relationships within the former Regina Qu'Appelle Health Region and the community.

Other Information

  • This is a community based position providing services in clinic and home settings. Travel required to various community sites, including rural, within the Primary Health Care Networks (Regina). Home base for this position is: 2180 Victoria Avenue E
  • Scheduled hours of work may be reduced (if applicable) during the rotation stipulated when a statutory holiday is recognized during that period.
  • Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)
  • Must possess a valid driver's license and have a vehicle available for work.
  • Willing to travel to rural areas as assigned.
  • Travel allowance is provided.
