Job Description: With reporting responsibility to the Director, Geriatric Services, for Integrated Health Services, the Continuing Care Consultant is responsible for assisting with the provision of consultative services for clients with responsive behaviours across southern Saskatchewan Health Authority (the area previously known as the five southern health regions). The incumbent advises and recommends services that are designed to meet the needs of clients with behavioural and/or psychological symptoms of dementia (BPSD), acquired brain injury (ABI), developmental challenges, and mental illness. The incumbent provides consultation services for residents in long term care facilities, clients in the community, and patients in the acute care hospitals.

Human Resources Exemption: No

Experience

  • Minimum of three (3) years relevant long term care nursing experience
  • Experience with adult mental health and geriatrics

Education

  • Bachelor of Science in Nursing (BScN)

Other Education and Training

  • Nursing diploma is acceptable
  • Basic Life Support Certificate (BLS)

Other Information

  • Registered/eligible to be registered with RPNAS (Reg Psych Nurses Assoc of Sask)
  • Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)
  • Valid Class 5 driver's license
