Job Description:

Reporting to the Urban Primary Health Care Manager, the Primary Health Care Nurse is an integral member of the primary health care team, providing and coordinating initial, continuing and comprehensive nursing services to patients in the PHC networks. The Primary Health Care Nurse promotes quality health care outcomes working within established standards for case management practice. The Primary Health Care Nurse initiates and/or participates in community development initiatives and participates in the evaluation of Primary Health Care initiatives within the networks.

Education

  • Bachelor of Science in Nursing (BScN)
  • Basic Life Support certification
  • Current registration with the Saskatchewan Registered Nurses Association (SRNA)

Experience

  • Minimum two (2) years of recent nursing experience in a community based setting is required.
  • Previous experience in teaching, facilitation skills and leading group activities is required.
  • Experience in case management is an asset.

Knowledge, Skills & Abilties

  • Demonstrated knowledge of chronic disease prevention and management.
  • Demonstrated clinical competency including critical thinking skills and confidence to work autonomously.
  • Demonstrates sound evidence-based knowledge and judgment when making decisions about a client's nursing care.
  • Demonstrates knowledge of population health promotion theory and practice and ability to transfer this knowledge into practice.
  • Willingness to adhere to established Public Health Services clinical policies and procedures.
  • High level of accuracy is required in all aspects of assigned responsibilities such as health assessment, nursing diagnosis and treatment, immunization, and handling specialized equipment.
  • Knowledge of and sensitivity to the impact of social, economic, environmental and cultural issues on chronic disease.
  • Team leadership and facilitation skills including an understanding of the fundamentals of team dynamics.
  • Demonstrates the ability to manage conflict toward effective outcomes.
  • Effective communicator with active listening and motivational interviewing skills.
  • Demonstrates empathy, trust and respect in the provision of care.
  • Demonstrates ability to accept and implement change.
  • Knowledge and application of quality improvement principles, tools, and outcomes.
  • Strong computer skills.
  • Able to prioritize tasks and work under pressure.
  • Knowledge of the lean management systems.
  • Systems thinking approach to health care delivery.

  • This is a community based position providing services in clinic within the Primary Health Care Networks (Regina). Home base for this position is: 1504 Albert Street.
  • Must possess a valid driver's license. Travel may be required.
