Job Description: The incumbent is an expert clinical practitioner who integrates experience, skills and knowledge from nursing theory and principles of adult learning into the provision and management of patient care, assessment of needs, clinical coordination, clinical problem-solving and education. The Transplant Coordinator facilitates and coordinates activities, clinical services and resources that support patient's pre and post-transplant care. The Transplant Coordinator is also responsible for coordinating living kidney donation. The Transplant Coordinator role includes "on call" responsibilities for the purpose of coordinating the transplant process and after hours care of transplant recipients and support to health care professionals as assigned by the Manager.

Human Resources Exemption: No

Experience

  • Minimum 5 years recent acute nursing experience
  • Experience in critical care is an asset
  • One (1) year of transplant and organ donation experience an asset

Education

  • Bachelor of Science in Nursing (BScN)
