Job Description

Reporting to a Primary Health Care Manager, the Staff Pharmacist is committed to the integration of knowledge and skill in the provision of quality, client-focused pharmacy services and the ongoing development and optimization of contemporary pharmacist directed patient care activities in a primary health care community multi-disciplinary team environment within the Regina Qu'Appelle Health Region. As an integral member of the Primary Health Care Team, the incumbent is responsible for chronic disease counselling, medication-related needs of patients, the provision of drug information to physicians, other health care professionals and the patient, and will serve as a preceptor to other pharmacists, pharmacy residents, and pharmacy undergraduate students, as required. The incumbent will also be involved in multi-disciplinary education and research. Working as part of this multi-disciplinary care team, the pharmacist must have the ability to establish effective, trusting working relationships with patients, healthcare teams and other resource providers. The pharmacist will assist in the planning and development, implementation, coordination, and evaluation of chronic disease programming. The successful candidate will be required to assist the team in collecting, analyzing and reporting data. The goal is to support clients in community so that they can successfully self-manage and lessen their dependency on our acute system.

Education
  • Bachelor of Science Degree in Pharmacy.
  • Certificate of completion of an accredited Hospital Pharmacy Residency Program or equivalent experience.
  • Membership in the Canadian Society of Hospital Pharmacy (CSHP) is an asset.
  • Collaborative Prescribing Certification required.

Experience

  • Specialized directly related experience is an asset.
  • Requires strong computer skills in order to work within all computer systems accessed for client care by the PHC Team.

Licenses

  • Saskatchewan College of Pharmacy Professionals (SCPP)
  • Valid Class 5 driver's license

Other Education and Training

  • Entry level PharmD is acceptable
