Job Description

As an integral member of the Primary Health Care Team this individual will use the customer centered approach to facilitate behavioral health changes through screening, assessments, brief interventions and education. A primary function of this individual will be to hear the customers voice and assist them in achieving their health needs through the development of strong relationships and individualized holistic care plans.
Working in a multidisciplinary environment including, but not limited to, Primary Health Care Team members: Family Physicians, Nurse Practitioners, Primary Health Care Case Manager, Home Care, Emergency Medical Services, Pharmacy, Therapy Services, Public Health, Dieticians, psychiatrists, Psychologists, Social Workers, Addiction Workers, and visiting specialists as this position requires networking with other professionals and agencies within the community.

Education
  • Recognized Bachelor Degree in Social Work is required.
  • Must have current practicing registration with Saskatchewan Association of Social Workers (SASW).

Experience

  • Three years' Mental Health experience in the past five years which includes community mental health experience working with clients with mental health and addiction issues.
  • Advanced classes in social work or psychology would be an asset.

Knowledge, Skills and Abilities

Counselling skills and extensive experience in a community outpatient setting. Assessment and intervention skills, good judgment. Interpersonal and communication skills to work effectively in a team setting. Good written skills to keep accurate records. Working knowledge of the Mental Health Act and Health Information Protection Act. Brief treatment modalities (MI, CBT, BI). Experience with Microsoft Office and its applications Word, Excel, Outlook. Understanding of the Lean Management System. Basic computer skills.

Other Information

  • Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement. HSAS seniority within the Saskatchewan Health Authority will be considered under the Selection Criteria.
  • This position is part of Primary Health Care Services and can be assigned to one of the Primary Heath Care Networks. This position may be required to provide services in a variety of settings including community meeting places, clinic settings or within a private residence including home visits. Home base is 1504 Albert Street
  • Must possess a valid Saskatchewan driver's license. Some assignments may require access to a vehicle for work.
