Job Description: The overall function of the assessor/coordinator (client care coordinator) position is to provide case management services which includes assessment, planning, monitoring and evaluation. The client/patient access services is primarily responsible for a single entry access and ongoing coordinator for a variety of community based services within SHA (Saskatoon).

Education

  • Bachelor of Social Work (BSW)

Other Education and Training

  • Bachelor of Occupational Therapy is acceptable
  • Bachelor of Nursing is acceptable
  • Bachelor of Nutrition is acceptable
  • Bachelor of Physical Therapy is acceptable

Knowledge and Abilities

  • Knowledge in the SHA, community resources and applicable programming
  • Assessment knowledge

Other Information

  • Registered/eligible with the Saskatchewan Association of Social Workers (SASW)
  • Registered/eligible for registration with the appropriate licensing body
  • Preference to HSAS members as per the SAHO/HSAS Collective Agreement
