Salary or Pay Band: Pay Band Assessor Coordinator - Degree $36.721 to $44.902

Job Description: Under the direction of the Manager, the Assessor Coordinator- Degree works with the Children's Interprofessional Practice Team. As a member of the Children's Interprofessional Practice, the Assessor Coordinator- Degree provides intake and system navigation to clients and to agencies, by providing screening, assessment, consultation, data collection, organizing and communication referrals, as well as family and community education. The Assessor Coordinator- Degree acts as a liaison and advocate for clients/patients, ensuring that clients with the greatest need have access to service in a timely manner. The incumbent will have a strong demonstrated knowledge of the social determinants of health, and their impact on health outcomes. The incumbent will work with Feeding and Swallowing providers including, but not limited to, speech language pathologists, nursing, psychology, social work, community program builders, Nurse Practioners and senior staff to triage referrals.
