Job Description: Through education and awareness, testing, outreach, and case management activities, the Communicable Disease Outreach RN will work to bridge health services on and off reserve for people living at risk or who have contracted HIV, Hep C, or other STBBIs. The Communicable Disease Outreach RN will have a strong background in sexual health with demonstrated experience in HIV and Hep C case management; excellent interpersonal skills and will embrace the philosophy of serving our communities as a staff member of the Saskatoon Tribal Council. The Outreach RN will have experience in working with First Nations communities and be knowledgeable about First Nations culture and traditions. The outreach nurse will work in conjunction with the First Nations Inuit Health Branch to provide client care.

  • Assist and support health services in the member communities and at the STC Health Centre, with a focus on HIV, Hepatitis C, STBBIs, sexual health, and harm reduction through, orientation, education, testing, linkages to care, case management, and data collection;
  • Collaborates with the STBBI team in the provision of harm reduction specific education for staff and clients in urban and rural communities;
  • Provide assistance and support to the communities harm reduction and sexual health programs and delivery of these services;
  • Community engagement and development by working extensively with urban and rural, indigenous and non Indigenous, communities to improve 90/90/90;
  • Provide assistance and support to the Community Health Nurses/ STC Health Centre Nurse in the area of case management including testing and linkages to care and treatment;
  • Collaborate with health professionals where appropriate through consultation and case conferences to evaluate clients' medical or physical condition and to assess client needs;
  • Support the review /update/development of the HIV Nurse orientation packages;
  • Make recommendations and assist with the development of program-related policies and procedures;
  • Monitors the effectiveness of programs & services and makes recommendations for improvement;
  • Conduct community forums and participate in public education activities such as Health Fairs and Treaty Days;
  • Monitor, complete quality controls, and maintain Saskatchewan Laboratory Licensing standards for STC and communities;
  • Participate in STC Accreditation and maintain best practices and quality improvement; Support STC and communities accreditation;
  • Assist and Support OH&S within STC and member communities;
  • Effective use of audio-visual media, advanced technologies, and community networks to communicate information and deliver the program objectives;
  • Utilize appropriate communication methods for interacting sensitively, effectively, and professionally with persons from diverse cultural, ethnic, socioeconomic, and educational backgrounds of all ages and lifestyles;
  • Define, assess, and understand the application of the determinants of health;
  • Establish and maintain professional linkages with key stakeholders;
  • Utilize leadership, team building, negotiation, and conflict resolution skills to build community partnerships; Identify community assets and available resources;
  • Perform other related duties as defined and assigned by the reporting Director or designate on an as and when required basis.

Education Licenses:

  • Bachelor of Science in Nursing;
  • Minimum of 2 years of nursing experience
  • Clinical experience in administering point of care testing, venipuncture, and immunizations is preferred.
  • Experience in Community Health Nursing is an asset.

Other Education and Training:

  • Knowledge in the areas of HIV, HCV, STI's and harm reduction;
  • Demonstrated case management practices and experience in conducting case reviews are essential.
  • Demonstrated knowledge and understanding of multi-cultural issues, addictions, mental health issues, family violence, sexual health, blood-borne infections (HIV/AIDS, STI's and Hepatitis C), and issues related to the impact of systemic challenges.
  • Highly self-motivated, energetic, organized, and detail-oriented with an attitude for continuous improvement;
  • Strong verbal, presentation, written, and interpersonal skills are required;
  • Ability to work independently and as part of a multi-disciplinary team;
  • Professional presentation, writing, and technical skills for academic and amateur audiences;
  • Good working knowledge of PC software and presentation applications (MS Office applications, internet, database familiarity);
  • Knowledge of audio-visual equipment usage;
  • Knowledge of the political, social, and economic objectives of the Saskatoon Tribal Council and of the Dakota, Cree, and Saulteaux cultures will be considered an asset.

Other Information:

  • Moderate risk of exposure to disease or illness through the provision of client interaction;
  • Exposure to clients who may have impaired judgment due to emotional distress, mental illness, or intoxication;
  • Must be willing to work flexible hours as assigned by management, which may include some evenings and weekends;
  • Travel within STC communities and the assigned areas of the program;
  • Must possess a valid Saskatchewan driver's license, a reliable vehicle, and meet STC's insurance requirements;
  • Must provide a current, original, Canadian Criminal Record Check (CPIC) and vulnerable sector check as a condition of employment.
