Job Description: Under the direction of The Home Care Manager, The Clinical Coordinator: ensures the provision of safe, comprehensive care in multiple work sites, meeting client and staff needs through coordination and communication within a large geographic area (both urban and rural), and establishing collaborative working relationships within the office as well as the multidisciplinary team members situated in a variety of locations. Provides functional direction and guidance to all staff (RN'S, LPN'S AND CCA'S) in initiating and obtaining the goals of the client care plan and provides direct client care when required. Supports families in crisis situations and other service providers or family members and acts as a client advocate. In conjunction with evening/weekend manager, facilitates planning and assignment of work, as well as coordination of service delivery on designated shifts, including screening and processing of referrals. Ensures completion of staff work records for payroll purposes for their shift. Ensures a collegial, collaborative working environment where creative problem solving and troubleshooting of service scheduling is communicated to all involved both client and staff in a respectful and clear manner. Works with The Clinical Educator to assess educational needs for evening staff. Participates in educational updates and/or certificates, i.e. home IV therapy program (H.I.T.P.) and participates in studies and research projects with the home care program, as required.
